What is a Geriatric Care Manager?
We

ourselves "Your Professional Relatives."
We specialize in assisting elderly adults
and their families with long-term care arrangements.
Assessment
We
begin all our services with a comprehensive
assessment of the elderly individual's situation,
which reviews
- Medical
history--diagnosis, illnesses, surgeries,
and medications
- Family,
friends, and others who are the elder's
support system
- Social
and occupational history
- Spiritual
and emotional well-being
- Home
safety
- Financial
resources, income, and expenses
Home
Care
We
help seniors stay at home as long as possible
by arranging services for them. Using home
health agencies, we obtain people to help
with cooking, housekeeping, shopping, and
transportation. We look in on our clients
at home weekly or biweekly to make sure they
are getting the services they need.
Placement
When
our clients reach a point when they cannot
be safe at home or it costs too much to pay
for home care, we help them find the best
senior apartment communities, assisted living
facilities, or nursing homes that are affordable
and will meet their needs. We take pride in
maintaining up-to-date information on all
residential communities and facilities in
Karachi There are very few assisted living
facilities and 60 nursing homes in Karachi!

We routinely monitor all essential features
of these facilities: licenses and ratings,
complaints or citations, ownership and administrative
staff, employee stability and turnover, services
provided, costs, and admission processes.
Our extensive database saves elders and their
families countless hours and stress in finding
the right home.
We have found that choice of residence is
determined by the elder's medical needs, family
and friends support system, geographic location,
and financial resources. Here is a sample
of some issues we review in our assessment:

he or she stay home alone or with assistance?
- Is
there a problem with taking medications
in the right amount at the right time?
- Are
there frequent falls at home?
- How
is his/her eyesight, hearing, mobility,
and ability to drive?
- Is
he or she depressed or showing signs of
increased sleep or inactivity?
- Who
visits the elder or helps with important
decisions?
- Do
family and friends live close to the elder?
- Has
the elder signed a living will, durable
power of attorney, or healthcare surrogate
form?
- What
is his/her monthly income? (Social
Security, pension, annuity, interest, dividends)
- What
are the monthly expenses? (mortgage
or rent, car maintenance, insurance, Medicare,
HMO premium)
Doctor's
Visits
We take clients to the doctor or
visit them in the hospital. We discuss
their symptoms, possible treatments, and medications
with doctors and nurses. Some of our
clients don't remember what to do for follow-up
care very well or get their medications confused.
Their families really appreciate our being
there to hear it correctly from the doctor
or pharmacist. One of our nurses usually goes
with a care manager to do a new client assessment.
Our nurses also review medical records and
prepare facility admission forms for the doctors
to complete.
Moving
Your Elderly Loved Ones to Karachi from Other
Places
We are experienced in finding the
right placement in Karachi facilities for
elders who live in other places. Working
with geriatric care managers and licensed
nurses all over the country, we are able to
assess the elder's needs and work with the
elder's physician and family to choose the
most appropriate residence. We even help with
moving and decorating!
Long
Distance Caregivers
We have a number of elderly clients
who live here but their families are out of
Pakistan. We provide much needed security
for the elders and relief from family worries
that someone is taking care of their loved
ones. We provide monthly written reports and
frequent telephone calls to family members.
We also help by serving as a local contact
or go-between for the elder's physicians and
out-of-city families. In some cases, we are
alternate healthcare surrogates if family
members cannot be reached during emergencies.
